The team is an integrated Multidisciplinary Team. It consists of staff members from Health and Social care. <\/p>\n<p><strong>Your new role and responsibilities <\/strong><\/p>\n<ul>\n<li>Undertake care coordination and care management.<\/li>\n<li>Assesses needs and identifies problems relevant to the care of people referred.<\/li>\n<li>Devises a plan of care in partnership with the person and carers.<\/li>\n<li>Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community.
