4 roles<\/strong><\/p>\n These are to add to the Hospital SW Team, split between the HomeFirst Team, and the D2A team.<\/p>\n The hospital social care team works primarily on completing assessments for individuals currently in a hospital setting, to put in place the most effective service to enable the individual to be discharged safely, either to their own home or to a residential \/ nursing home dependent upon the person\u2019s needs.<\/p>\n The assessments are split across three areas:<\/p>\n – Home First (team based at Longton Cottage Hospital)<\/p>\n – Community Hospital\/D2A (team based at Haywood Hospital\/Civic)<\/p>\n – Integrated Discharge Hub (team based at Royal Stoke UHNM)<\/p>\n AND<\/em><\/p>\n 2 roles<\/strong><\/p>\n To work alongside the urgent care at home team and commissioned domiciliary care providers to provide a 7 day rapid response assessment of individuals residing in the city. <\/p>\n The service is a multi-disciplinary response to prevent admission to hospital as well as a reliance on long term care.<\/p>\n You will need to be:<\/p>\n Please contact me asap to discuss in more detail.
