These are to add to the Hospital SW Team, split between the HomeFirst Team, and the D2A team.
The hospital social care team works primarily on completing assessments for individuals currently in a hospital setting, to put in place the most effective service to enable the individual to be discharged safely, either to their own home or to a residential / nursing home dependent upon the personβs needs.
The assessments are split across three areas:
– Home First (team based at Longton Cottage Hospital)
– Community Hospital/D2A (team based at Haywood Hospital/Civic)
– Integrated Discharge Hub (team based at Royal Stoke UHNM)
AND
2 roles
To work alongside the urgent care at home team and commissioned domiciliary care providers to provide a 7 day rapid response assessment of individuals residing in the city.
The service is a multi-disciplinary response to prevent admission to hospital as well as a reliance on long term care.
You will need to be:
- Qualified Social Worker
- Fully Social Work England registered
- Post-qualified experience in an adult social care statutory setting
