The Supported Housing Team here at Havebury is looking for a new Support Housing Advisor to join our incredible Team, to help support our residents further! Your role is to identify support that our tenants may need (general needs / sheltered tenants or any other customer as appropriate) to improve and support them in their own homes, in line with our supported strategy. This may be by assessing needs, creating support plans and referring and signposting to internal and external services as required or simply signposting or administering financial support as part of our support fund policy.
Please note this is a part-time role of 2 days per week (14.8 hours).
Requirements
Outline of Key Responsibilities…
- Provide support and advice to tenants and customers as necessary across all contact channels , forging links with external statutory and voluntary agencies to access services to preserve independent living e.g. Meals on Wheels, home care, etc and ensure communications are documented
- Complete support plans / PEEPs and review at agreed timescales, including early review if there is a significant event which indicates it is necessary. Ensure this information is recorded appropriately and shared with the tenant/ customer
- Carry out fire safety or other checks and ensure tenants are fully aware of all procedures in the event of a fire
- To be a presence at schemes (general needs and independent living) on a peripatetic basis to promote and support independence, signposting and referring for additional support as required
- Respond to emergency calls/enquiries when on duty, escalating accordingly and notifying next of kin as necessary
We are looking for someone who has…
- Experience of working in support services either with a voluntary or statutory organisation
- Knowledge of physical, psychological and social needs and how these can be supported
- Familiar with Microsoft resources and ability to learn internal systems
- Confidence to tackle difficult emotive situations
- Awareness of social services and other agencies
- An understanding of Safeguarding responsibilities
- Current Driving Licence and access to own vehicle
